New Patient Information

What to Expect for Your First Visit

When you arrive you will be greeted by the receptionist and checked in. She will hand you the New Patient Form along with our HIPPA Policy and will take your license and insurance card to make a copy. When you’re finished, the receptionist will take your completed forms to establish a patient record. While your patient record is being processed, you can relax in our waiting room and watch tv.

Then, one of our medical assistants will come out to bring you back to a treatment room. If we feel you need x-rays, we may take those first so our doctor can review them before seeing you. Both socks and shoes will be removed and you can make yourself comfortable in our treatment chair. Please notify our medical assistants of any allergies.

You will then meet one of our doctors, where you will have the opportunity to discuss your foot issues and ask any questions you may have. Treatment options will be discussed with you. You may need a follow up visit to be scheduled to monitor ongoing conditions. If the doctor feels they need an additional test (for example, an MRI or CT Scan), we will arrange to schedule your tests and follow up appointments. Please expect a minimum of a half hour in our office.

Health Insurance Coverage / Durable Medical Equipment

Our office accepts Medicare and a variety of HMOs, PPOs, and other health plans. Please contact our office to verify participation with a specific carrier. We make insurance easy! If you do not have health insurance or we do not participate with a particular insurance carrier, we can do self-pay arrangements. For more information click here.
